**Probands** Recruitment went from 2003 to 2006. The physical baseline evaluation went from May 2003 to June 2006, and the psychiatric baseline evaluation went from January 2004 to May 2009. Participants were followed up for the physical evaluations between April 2009 and September 2012, May 2014 and April 2017, April 2018 and March 2021, and have been followed since January 2022 for the fourth follow-up. Participants were followed up for the psychiatric evaluations between January 2004 and May 2009, April 2010 and January 2014, November 2014 and July 2018, and April 2018 and September 2021, and have been followed up for the fourth wave since October 2021. Participants could participate in physical, psychiatric, or both evaluations. **Metabolic Probands** Recruitment for physical and psychiatric baseline evaluations went from 2010 to 2014. Participants were followed up for physical evaluations between May 2014 and April 2017, April 2018 and March 2021, and have been followed since January 2022. Participants were followed up for psychiatric evaluations between November 2014 and July 2018, April 2018 and September 2021, and have been followed since October 2021. **Relatives (of the probands)**
Offspring younger than 18 years and spouses of the probands of this cohort were recruited between 2007 and 2012 for the psychiatric baseline evaluation and were thereafter followed up every 3 years. Adult first-degree relatives were recruited between 2007 and 2012 for psychiatric and physical evaluations and were thereafter followed up every 3 years. Offspring aged 18 to 35 were recruited from 2019 to 2023 for physical and psychiatric evaluations and have been followed up every 3 years since 2019. **Relatives (of the metabolic probands)** Offspring and spouses of the probands were recruited between 2010 and 2014 for the baseline psychiatric evaluation and have since been followed up every 3 years. Starting in 2014, spouses and adult offspring have also undergone physical evaluations every 3 years.

**Metabolic Probands** Metabolic cohort: Given the relative rareness of severe mental disorders in the general population, a cohort of psychiatric patients with schizophrenia, bipolar disorder or major depressive disorder originally enrolled in a study on metabolic complications under psychotropic drug treatment was recruited during the first follow-up of the population-based cohort. These patients were in the age range of the original cohort, fulfilled the same inclusion and exclusion criteria, had the same physical and psychiatric assessment as the original cohort and were followed-up in parallel. **Relatives of the Probands** The objectives of the recruitment of offspring younger than 18 years and spouses from 2007 to 2012 were to prospectively study the associations of parental disorders and environmental factors with the risk of onset of mental disorders during adolescence and young adulthood. The objectives of the recruitment of adult first-degree relatives and spouses from 2007 and 2012 were to determine the patterns of familial aggregation of mental disorders, to assess spouse similarity and assortative mating and to explore the mechanisms underlying the associations between mental disorders and cardiovascular risk factors through the investigation of patterns of aggregation and cross-aggregation of these disorders. The objectives of the recruitment of adult offspring from 2019 to 2023 were to assess mental disorders and obesity markers in young adulthood as well as their associations, to identify factors involved in these associations and to determine the genetic, familial and environmental determinants of the parent-offspring transmission of mental disorders and obesity markers. **Relatives of the Metabolic Probands (only psychiatric evaluations)** The objectives of the recruitment of offspring younger than 18 years and spouses from 2010 to 2014 were to prospectively study the associations of severe parental disorders and environmental factors with the risk of onset of mental disorders during adolescence and young adulthood.Timeline

A random sampling procedure based on a complete list of the inhabitants of Lausanne aged 35 to 75 years, according to the civil register of the city, was used. Subjects who agreed to participate were contacted by phone within 14 days. Subjects who did not answer the first invitation were sent a second invitation letter. If no answer was obtained, participants were contacted by phone. After completion of the physical evaluation, participants were contacted by the psychiatric team by letters and phone calls to elicit consent for the psychiatric part.

The cohort is composed of 52.6% of women and 91% European descent. The cohort did not significantly differ from the population of Lausanne with respect to the sex distribution, but the two youngest and the two oldest age bands were underrepresented.

During the psychiatric evaluations between 2007 and 2012, all participants (probands) from the original cohort (probands) were systematically asked, whether they would allow the study to contact their first-degree relatives. If they agreed, written consent was requested for each relative in order to give the study permission to contact this person and to complete a brief self-report pedigree including the addresses and phone numbers of their relatives. These relatives were then invited by letter to participate. Those who agreed were contacted by a psychologist by phone, who provided further information on the study and sent them the detailed information letter and consent forms.

During the third and the fourth physical and psychiatric follow-up evaluation, all participants (probands) of the original cohort were systematically asked whether they would allow the study to contact their offspring in the age range between 18 and 35 years. If they agreed, written consent was requested for each offspring to give the study permission to contact this person. These offspring were then invited by letter to participate. Those who agreed were contacted by a psychologist or study nurse by phone, who provided further information on the study and sent them the detailed information letter and consent forms. Those who completed first the psychiatric evaluation, were contacted by the somatic team in a second step and vice versa.
