Determinants of Community COVID-19 Transmission: Learning from the Hutterites
The overall goal of this study is to understand the community determinants of COVID-19 transmission.
Specific objectives are:
* To determine if pre-existing SARS- CoV-2 antibodies in children prevents subsequent infection in others (i.e. is there a herd effect),
* To define longitudinal SARS-CoV-2 viral load in children and its relationship to colony transmission,
* To determine the role of physical distancing in COVID-19 colony transmission,
* To assess the diagnostic accuracy of respiratory and serological specimens that are being used to detect COVID-19,
* To determine the effect of influenza on SARS-CoV-2 infection at the individual and community level,
* To assess re-introduction of SARS-CoV-2 viruses in colonies by whole genome sequencing.

In the main phase, colonies or participants are followed-up biweekly over 2 to 6 month (surveillance), at mid-point and at the end of the trial. In the extension phase, enrolled colonies and new colonies/participants are followed-up weekly for up to 3 months (active surveillance) and at the end of the trial. In the Omicron variant extension phase, enrolled colonies or participants are followed-up weekly for up to 6 months (active surveillance), at mid-point and at the end of the study.

This study was conducted in 20 Hutterite colonies in which the mean population size of the study colonies is 103 (range 62 to 143) members, with on average 24 households per colony.
