Long-term Care, Immunity, and Frailty in Elderly COVID-19
The primary objective of this study is to prospectively evaluate global and SARS-CoV-2 specific immunity in COVID-19 exposed uninfected, COVID-19 recovered and COVID-19 unexposed long term care facility (LTCF) residents before, during and after SARS-CoV-2 vaccination.
The secondary objectives are to:
1. Measure the breadth, amplitude and frequency of COVID responses in LTCF residents;
2. Compare anti-COVID adaptive responses in cytomegalovirus (CMV) negative and CMV positive LTCF residents as well as those with and without seasonal coronavirus immunity;
3. Describe humoral and cell mediated anti-COVID vaccine induced immunity;
4. Understand relationships between frailty, CMV co-infection and baseline immune phenotype in immune resilient (exposed uninfected) LTCF residents;
5. Describe clinical outcomes with respect to baseline frailty in LTCF associated COVID infection;
6. Biobank samples from a low-prevalence SARS-CoV-2 negative wave 2 longitudinal immunity cohort that can be used to assess difference in frailty or immunity or co-infection parameters between high, medium and low prevalence LTCF populations.

After the baseline visit, participants were screened before vaccination and then followed-up at 5 timepoints: pre-boost (2-4 weeks post first dose), 1 month, 3 months, 6 months and 1 year post-boost.

The population is composed of highly exposed to COVID-19 virus but uninfected residents of two long-term care facility (LTCF) campuses; Northwood campuses and the St. Vincent campus, in Nova Scotia, Canada.

Resident of a long-term care facility;
Able to provide consent either directly by the participant, or surrogate in the event of a medical emergency and/or subject who is not capable of providing informed consent;

A subset of the Wave 2 low prevalence cohort (N=60) will be examined for T and B cell immunity and the remainder will be biobanked for future studies aimed at understanding immunity and biomarkers in vulnerable older persons.
